​​OUR GUARANTEE

​

For a period of three full years, Archline Dental Arts guarantees:

  • The workmanship of all new fixed restorations.

  • That all new fixed restorations will fit the master die or master model, provided that adequate reduction of the prepared tooth was completed according to the manufacturer’s recommendations.

​

WHAT ARCHLINE DENTAL ARTS COVERS

​

Archline Dental Arts, at its sole discretion, will either repair, replace, or issue a credit for the invoiced amount of a defective restoration.

  • All credits/refunds are limited to the original invoice amount.

  • Archline Dental Arts reserves the right to determine if the guarantee is applicable to a specific case.

WHAT ARCHLINE DENTAL ARTS DOES NOT COVER

​

  • Cash refunds for completed work.

  • Costs associated with the removal or reinsertion of restorations.

  • Incidental or consequential damages, including patient inconvenience, lost chairtime, or additional clinical costs.

  • Failures resulting from cementation or bonding issues.

  • Failures due to accident, neglect, abuse, changes in tissue or bone structure, or improper dental hygiene.

  • Restorations that have been altered, adjusted, or modified by external parties in a manner inconsistent with laboratory instructions or standard dental practices.

  • This guarantee is void if the original preparation, model, or accompanying documentation demonstrates inadequate reduction or improper margin placement that prevents proper seating.

PAYMENT TERMS

​

  • All accounts are payable within 15 days of the statement date.

  • Statements are issued on the 1st of every month.

  • Accounts not paid within the stated terms are subject to a 2% monthly finance charge on the outstanding balance.

  • This guarantee is suspended and unenforceable for any account that is past due. The guarantee will be reinstated upon full payment of the outstanding balance, provided the failure occurred within the original three-year period.

  • Prices are subject to change without notice.

PRIVACY POLICY

INTRODUCTION

​

Archline Dental Arts ("Archline," "we," "us," or "our") is a California-based dental laboratory committed to protecting the privacy and security of the information we manage. This Privacy Policy outlines how we handle Protected Health Information (PHI) in accordance with the federal Health Insurance Portability and Accountability Act (HIPAA) and Personal Information (PI) in compliance with the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act (CPRA).

HIPAA COMPLIANCE

​

As a dental laboratory, Archline acts as a "Business Associate" to licensed dentists and dental practices. We receive PHI via digital work orders, physical prescriptions, electronic transfers (intraoral scans), and physical impressions.

We use and disclose PHI without specific patient authorization only as necessary for the following purposes (TPO):

  • Treatment: To manufacture, customize, and adjust Zirconia dental restorations as ordered by the referring clinician.

  • Payment: To bill and collect payment for our services from the dental practice or third-party payers.

  • Healthcare Operations: For internal quality assurance, laboratory management, and professional training to ensure the highest standards of restoration fit and finish.

3CCPA/CPRA COMPLIANCE

​

This section applies to the Personal Information of California residents who are not patients. This includes our dental clients, their staff, job applicants, and website users.

  • No Sale of Data: We do not Sell or Share Personal Information as defined under the CCPA/CPRA.

  • Service Providers: We disclose PI only to trusted Service Providers under strict contracts that prohibit any use of the data beyond providing the specified service.

CALIFORNIA RESIDENT RIGHTS (CCPA/CPRA)

If you are a California resident, you have the following rights regarding your PI (excluding PHI):

  • Right to Know/Access: The right to request the categories and specific pieces of PI we have collected.

  • Right to Delete: The right to request the deletion of PI, subject to legal and regulatory retention requirements.

  • Right to Correct: The right to request the correction of inaccurate PI.

  • Right to Non-Discrimination: We will not discriminate against any individual for exercising their privacy rights.

DATA SECURITY AND RETENTION

​

Archline Dental Arts employs industry-standard administrative, physical, and technical safeguards consistent with the HIPAA Security Rule. This includes encrypted digital transfers and secure physical facility protocols.

Data is retained only for as long as necessary to fulfill the purpose of collection, or as required by California and Federal law regarding medical and business record retention.
